Outreach and Home Visits are services that we provide as part of our Family Support.
A home visit can be made with a family’s permission when a referral has been made by a health, education or other professional involved with that family.
We can visit any family within our reach area (please see the map for our reach area).
In exceptional circumstances we may visit families who are not within our reach area.
We also provide outreach which is carried out by the Healthy Living Outreach Staff and Home Visits from the Nursery School EarlyStart Staff.
